<image:title>Blog - Examples of Enhancing Treatment & Care with Hyperbaric Oxygen Therapy (HBOT): Adjunctive HBOT in the Management of Chronic Rhinitis - Physical exam was unremarkable except for a bilateral green purulent nasal discharge and episodic sneezing. MRI (shown left) revealed diffuse soft tissue density bilaterally in the rostral nasal cavity without turbinate destruction. Rhinoscopy revealed a normal posterior nasopharynx, with soft tissue swelling and purulent discharge in both nasal passages. Histopathology of biopsy samples obtained from the nasal mucosa revealed bilateral chronic lymphoplasmacytic follicular rhinitis, with focal areas of neutrophillic infiltrates. Culture of the nasal mucosa biopsy specimens yielded Pseudomonas.</image:title>

<image:title>Blog - Examples of Enhancing Treatment & Care with Hyperbaric Oxygen Therapy (HBOT): Adjunctive HBOT in a Case with Complete Global Ischemia (CGI) - A 7-year-old F/S canine was referred as an emergency transfer with an endotracheal tube in place. The patient had been induced with propofol after premedication with acepromazine and glycopyrolate. Tremors, twitching, and dyspnea were noted soon after the propofol was administered, and the anesthetic procedure was aborted. However, efforts to extubate the patient had resulted in labored breathing with cyanosis, so the patient was re-intubated and transported to the referral center.</image:title>

<image:title>Blog - Emergency MRI Reveals ANNPE: Kali’s Rapid Road to Recovery - The day before New Year’s Eve, Kali, a sweet 4-year-old mixed-breed dog, experienced a sudden, scary injury while playing outside. In an instant, she cried out and collapsed — suddenly unable to use her back legs</image:title>
<image:caption>An emergency MRI revealed the cause immediately. Kali had an Acute Non-Compressive Nucleus Pulposus Extrusion (ANNPE) at T12-13 — otherwise known as a traumatic disk. ANNPE, or traumatic disk, is a condition where a tiny piece of normal, hydrated disk material 'explosively' ruptures into the spinal canal.</image:caption>

The image (B) on the right is a cross-section MRI at T12-3, which clearly shows a defect within the spinal cord representing the hemorrhage, swelling, and bruising from the tiny disk passing through the spinal cord. There is no compression from this ANNPE.</image:caption>

<image:caption>You are treating a very large Mastiff for marked spinal stenosis presenting with cauda equina syndrome. A dorsal laminectomy has been performed at L7-S1, and you decide to employ adjuvant HBOT in light of the severe swelling of the nerve roots discovered at surgery. This dog is very gentle, but repeatedly removes his anti-static bracelet when you try to place it. Please describe another option for assuring adequate grounding during an HBOT session for this patient. ANSWER: To assure safe grounding, you may place one or two large grounding plates under the patient, and attach the ground wire to the grounding connection of the chamber or the chamber door. All bedding and towels must be 100% cotton.</image:caption>

Spontaneous respirations were present (image at left). The puppy had recurring semi-formed stools of a strong, sweet odor, containing a large amount of undigested grain and hay material. Mycotoxin intoxication was suspected. The owner also reported an empty vial of phenylbutazone was found in the barn, and that raccoons were recently suspected of breaking into various containers in the barn closet. Blood tests revealed mild increases in GGT and Na, with mild hypokalemia. Blood glucose levels were repeatedly normal. Supportive fluid therapy was continued. IV naloxone and oral acetylcysteine were given.</image:title>

<image:title>Blog - HBOT Safety/Review Questions #6 - You are treating a female spayed canine with marked azotemia and pyelonephritis. Urine culture by cystocentesis has revealed an organism susceptible only to gentamycin, which is being given IV.</image:title>
<image:caption>You want to support GFR at all times. You are employing adjuvant HBOT, so you have the patient on intravenous fluids during her sessions. The patient was initially very depressed and moved about very little during her sessions.</image:caption>

<image:title>Blog - HBOT Safety/Review Questions #6 - ANSWER: This is not an emergency situation. You should first insert a sterile catheter plug into the IV "pass through port" on the chamber door, and thus stop the flow of oxygen out of the chamber. (See image.)</image:title>
<image:caption>You may then consult with the attending doctor and ask if it is appropriate to gradually end this HBOT session with a standard, slow decompresion over 10-15 minutes, then flush and reconnect the intravenous line connections and begin a new, abbreviated session to make up for the time remaining in the shortened session; or the doctor may decide to simply end this particular session and wait until the next scheduled HBOT session for this individual patient.</image:caption>

HBOT's vasoconstrictive action can reduce tissue fluid loss in severe thermal burns. HBOT's method of action in CO toxicity is to increase the half-life of CO from minutes to hours. HBOT's stimulation of angiogenesis is a key factor in treatment of radiation-induced delayed wound healing.</image:title>

The patient treated in the chamber just prior to this one had diagnoses of severe pancreatitis and ketoacidotic diabetes mellitus, and was on an IV infusion during the session with the line "passed through" the chamber door at an accession port. As you start the session on the wound patient a steady hissing sound of air flowing through the accession port begins and intensifies. What is the likely cause? Is this an emergency? <image:title>Blog - Rehab for Pets: Using Rehabilitation Therapy to Maximize Recovery - Acupuncture has been used in veterinary practice in China for thousands of years. Acupuncture is indicated for functional problems such as those that involve paralysis and pain. In small animals such as dogs and cats, insertion of acupuncture needles is virtually painless, and the pet is generally relaxed, even sleepy. We use both electro-acupuncture (shown at left) and traditional needling at VNIOC. Veterinary manipulative therapy is a hands-on approach used to assess and treat musculoskeletal and nervous system issues in animals. It aims to restore proper joint motion, reduce pain, and improve overall function by addressing imbalances in the spine, muscles, and nervous system. It is the veterinary equivalent of human chiropractic care.</image:title>

<image:title>Blog - Rehab for Pets: Using Rehabilitation Therapy to Maximize Recovery - Laser therapy, also known as photobiomodulation (PBM) or low-level laser therapy (LLLT), is used to manage pain, inflammation, and promote tissue repair using red and near-infrared light. This therapy stimulates cellular processes, leading to reduced pain and inflammation, and enhanced wound healing and tissue regeneration. It is non-invasive, pain-free and can help animals recover faster and regain normal function after injuries or surgery. Right: A dog undergoing laser therapy. Shockwave therapy is a non-invasive treatment that uses high-energy sound waves to stimulate healing and reduce pain in pets. This non-surgical therapy which generally can be provided without sedation, can promote cell regeneration, increase blood circulation, and break down calcifications, aiding in tissue repair. The treatment is relatively quick, typically lasting only a few minutes, and often requires one to three sessions spaced a few weeks apart.</image:title>

<image:title>Blog - Rehab for Pets: Using Rehabilitation Therapy to Maximize Recovery - Underwater treadmill therapy is a versatile therapy used for various conditions in dogs and cats. It uses a treadmill submerged in water to help animals recover from injuries, manage chronic conditions, and improve mobility. Exercising in water provides buoyancy and support, reducing stress on joints and muscles. Hydrotherapy can improve muscle strength, cardiovascular fitness, and range of motion.</image:title>

<image:title>Blog - We've Got Tech: Advanced Neurosurgery Tools at VNIoC - The SonoCure Ultrasonic Surgical Aspirator® is a highly specialized tool that allows for targeted tumor removal without damaging surrounding neural tissue. As the first US veterinary clinic to obtain this equipment, we have extensive experience in using it to deliver outstanding outcomes for our patients. The SonoCure uses ultrasonic frequencies to basically emulsify the target tissues. The ultrasound waves cause cancerous cells to dissociate from one another while the device irrigates the area with sterile saline. The same handpiece then aspirates the cells/fluid back out of the surgical site. Because the device can be fine-tuned, it allows the surgeon to remove tumors with minimal effect on the adjacent tissues. It can even 'skeletonize' the blood vessels which massively decreases the intraoperative bleeding! We use our SonoCure for removal of brain tumors, spinal cord tumors, and other delicate procedures involving the central nervous system.</image:title>

<image:title>Blog - We've Got Tech: Advanced Neurosurgery Tools at VNIoC - Veterinary microsurgery is a general term for surgery requiring an operating microscope. At VNIoC we utilize the Atmos Neurosurgical Microscope to provide magnification and illumination in our neurosurgical patients to improve surgical exposure without creating large surgical fields. We find the Atmos Neurosurgical Microscope provides the perfect combination of light and magnification for our surgical cases. The LED lighting provides bright, balanced, and targeted illumination with no associated generated heat. The powerful objective lenses can increase magnification from 2.5 to 30 times. Additionally, the video feed allows us to share images in real-time or via video capture, making it an ideal tool for teaching and presentations.</image:title>

<image:title>Blog - We've Got Tech: Advanced Neurosurgery Tools at VNIoC - Intraoperative ultrasound (IOUS) is a dynamic imaging modality that provides interactive and timely information during surgical procedures. IOUS is critical in neurosurgery by localizing pathology (eg, the tumor), guide surface incisions fro deep lesion resection, and evaluate resection results in real-time. We use the Terason uSmart® 3200T Ultrasound System enables us to provide the highest level of intraoperative neurosurgery. Working with Animal Health Innovations, we have been able to utilize incredibly small US transducers that allow us to visualize brain tumors through a 8-15mm bone window. With this advanced technology, we are able to offer our patients a new level of excellence in veterinary neurology.</image:title>

<image:title>Blog - A Happy Tail, A Pawrents Perspective: Vida the Weimaraner - After an MRI showed a disc Alicia says was "totally obliterated," Alicia was given the choice of rest and steroid or surgery. She opted for surgery, saying, "Vida's too active a dog for prolonged rest!" Dr. Mannix and Dr. McDonnell performed the ventral slot surgery that showed the problem was worse that originally thought. After an overnight stay at Pet+E.R., Vida was up and walking, ready to come home. "She recovered beautifully and today is back to being the very active leader of our pack," according to Alicia (and as you can see in the photo at left, taken six months post-surgery). "We did have to confine her to one room and stay with her 24/7 at first so she wouldn't hurt herself. At first we used a sling, then a harness, just to be safe. "We're so glad we did the surgery! Vida now has a great quality of life. We're so grateful to the entire VNIoC team!"</image:title>

<image:title>Blog - Making Good Choices in Veterinary Anesthesia- Procedural Sedation - In our practice we have various needs for procedural sedation for short procedures such as radiology, BAER testing (Brainstem Auditory Evoked Response testing, AKA - hearing testing), and CT scanning, amongst others. We see a broad range of patients from healthy puppies to geriatric patients with multiple comorbidities. Let’s talk about some different scenarios where procedural sedation is needed, and what some protocols may look like for these cases.</image:title>

The patient had been treated with a tyrosine kinase receptor inhibitor for multiple recurring cutaneous mast cell tumors over the period of three months prior to referral for HBOT. A veterinary dermatopathologist interpreted the biopsies as multiple acute necrotizing dermatitis, canine toxic shock syndrome, and toxic epidermal necrosis (“TEN”). Culture of the skin biopsy revealed beta hemolytic strep sensitive to azithromycin. There was a marked (1.8 mg/dl) hypoalbuminemia. Treatment included azithromycin, HBOT, hydrotherapy, hand feedings, and prednisolone.</image:title>

<image:title>Blog - HBOT Safety/Review Questions #3 - The cool weather of autumn has finally arrived. When you removed the patient from the chamber today after a session you found dozens of "seed ticks" all over its coat, which you had not noticed when you wiped him down with a damp 100% cotton cloth prior to therapy. You immediately informed your supervisor, who sprayed down the inside walls of the chamber and bedding with a flea and tick spray product. You have more treatments scheduled today. Please explain how you would proceed from here.</image:title>

<image:title>Blog - HBOT Safety/Review Questions #3 - Cystocentesis had been performed when the initial attending doctor was unsuccessful in passing a catheter, and the bladder had ruptured during that procedure. The cat was initially extremely azotemic and hyperkalemic, and at emergency surgery the bladder wall was very distended, bruised, edematous, and appeared atonic. A Foley catheter was placed during the bladder repair, and a few days later a pharyngostomy feeding tube was placed due to anorexia and liver biopsy results which revealed marked hepatic lipidosis.</image:title>

You suspect sepsis, and recommend HBOT in addition to parenteral antibiotic therapy. The client is a pediatric neonatal ICU nurse at your local human hospital. <image:title>Blog - HBOT Safety/Review Questions #3 - The attending surgeon informs you that the stomach tissue appeared poorly perfused even after decompression and gastropexy, and there are currently runs of VPC's on the ECG. Review of the history reveals that the patient recently underwent a "limb sparing" procedure, and is being treated by an oncologist with chemotherapy for osteosarcoma. The surgeon is inquiring about the potential benefits of a short course of HBOT as adjuvant therapy for the ischemia-reperfusion issues associated with the GDV. You check the current/recent medication list. Which of the following is a relative contraindication for HBOT? Omeprazole Metaclopramide Cis-platinum Enrofloxacin</image:title>

<image:title>Blog - HBOT Safety/Review Questions #3 - An 8 year old canine has been referred for adjuvant HBOT in the management of non-healing foot wounds (of a six-year duration). You obtain current blood tests, biopsies and cultures, and prepare to begin therapy. The owner presents the dog for its first session with an air-filled "inner-tube" type of Elizabethan collar to prevent licking at the wounds and self mutilation. Please discuss the issue with this particular collar and HBOT.</image:title>

<image:title>Blog - Examples of Enhancing Treatment & Care with Hyperbaric Oxygen Therapy: Adjuvant Hbot In Sciatic Nerve Disfunction - Abnormal insertional activity on needle EMG (Image Right) confirmed a sciatic nerve lesion. An exploratory surgery at the FHO site revealed granulation tissue associated with the ostectomy site. The sciatic nerve was enveloped by this reactive tissue. Suture material from the previous surgery was removed. A Staphylococus species was cultured from the granulation tissue. The femoral neck excision was revised. A two-week course of enrofloxacin (based on sensitivity results) was begun, and a total of 7 HBOT sessions over 10 weeks were given. Marked improvement in gait was noted after the first three sessions, which were given within 48 hours post op. Progressive recovery continued. Normal right rear limb gait was noted on follow up 5 months later. Adjuvant HBOT has been reported to be effective in peripheral nerve injury in human medicine [1] and in some rabbit models of peripheral nerve trauma [2].</image:title>

He maintains a normal and steady PCV. You suspect sepsis. The fever is controlled with IV fluids and antibiotics. You want to add adjuvant HBOT. You review the thoracic radiographs (right). <image:title>Blog - What's the prognosis with back dogs? - Perception of deep pain - Top panel: Sam is relatively relaxed, laying down. He cannot support himself and drags his rear legs with his front legs. He will leak urine in his bed but his bladder is very big. When you tickle his paws or tail, he is not aware of the examiner. Bottom panel: He does not respond to any stimulation until our nurse uses a big hemostats and pinches his toe. Only then does he roll up, whines, and try to stop her from pinching any further by trying to bite.</image:title>

<image:title>Blog - MAXIMIZING MAX’S RECOVERY - When Dr. Bogen took Max in for the consultation, Dr. Archambault immediately noticed gait and reflex abnormalities, which led him to suspect a spinal cord problem. The MRI showed disc-associate Wobbler syndrome (DAWS) involving his C5-6 disk space, which caused compression and gliosis of the spinal cord. Dr. Archambault consulted with Dr. McDonnell on the case and a plan was proposed to perform cervical disk replacement using a titanium artificial disc developed by Dr. Filippo Adamo in California. This surgery shows excellent results with rapid recovery and return to activity. Replacement is superior to other surgical procedure such as a ventral slot or stabilization. More information regarding this surgery is at wobblersyndrome.com/adamo-spinal-disc/ which includes a short clip of the surgical procedure. “We can immobilize people with halos or other devices so they can heal after neck surgery. With dogs, it’s hard to keep them still, so the type of surgery and follow-up we do with humans won’t work,” explains Dr. Bogen.</image:title>

<image:caption>Dynamic CSM, also known as disc-associated Wobbler Syndrome, is a particularly devastating disease that affects the cervical and intervetebral discs, causing a narrowing of the cervical canal and compression of the spinal cord. The syndrome generally affects large breed dogs, especially Doberman Pinschers and Rottweilers, that are 4 to 8 years or older, but it can affect any size, breed or age of dog.</image:caption>

<image:title>Blog - SIX QUESTIONS DOG OWNERS ASK ABOUT BACK PROBLEMS AND PARALYSIS IN DOGS - This is Frank, he’s a 6-year-old Yorkie Mix. He is 6 years old and he came to VNIOC like this: completely paralyzed in his pelvic limbs; very painful; not able to urinate; and had gotten worse despite excellent medical treatment (crate rest, anti-inflammatories and pain medications). Two hours after admission, he had a spinal MRI that found a large T12-13 disk that compressed his spinal cord by more than 45%. He had a hemilaminetomy that same day to remove the compression, inspect his spinal cord and fenestrate his disk space. He went home less than 4 days later able to move his legs, stand and urinate on his own. Ten days after surgery, he is able to walk independently, is pain-free and expected to make a full recovery in another month. This is the outcome in 95% of dogs like Frank that present to VNIOC for intervertebral disk disease.</image:title>
