Request an MRI

  • MRI SCAN REQUEST

    This form should be submitted at least 24 hours in advance of the date that you are requesting. Incomplete packages (i.e. no blood work or chest radiographs) will not be reviewed until all information is provided. We advise that you do not confirm the scan with your client until the Imaging Team has confirmed availability and time with you. Specifics of your request, questions about positioning and scan time can be directed to the Imaging Veterinarian or the Technologist on duty. We will do our best to accommodate your requests, but same-day scans may not be possible due to pre-existing case load.
  • Please be as specific as possible. This will aid us in selecting the appropriate protocols and assist the radiologist when reading the scan.

    Please note that if you are requesting more than one site, the scans will be billed as additional studies. For example, a request for brain and neck MRI will be billed as two separate sites.
    Joint studies are performed individually in MRI and multiple cannot be scanned as one study. For example, a bilateral shoulder examination will be charged as two full studies. This is due to the patient repositioning and length of time required to scan.
